OFFICE POLICIES

Getting Ready for Your Visit

  • New Patients: Download and complete New Patient Packet  [Download Here]

  • Current Insurance card and required copay

  • Photo ID

  • List of medication with dosage and how you take them (include vitamins & supplements)

  • Medical records transferred to our office prior to your first visit. 

What Not to Bring to Your Visit

  • Please refrain from bringing young children to your appointment with the exception of newborns. This policy is to protect your child from harmful items in our exam rooms (bio-hazards and expensive medical equipment) and to protect our pregnant moms from common childhood diseases like chickenpox and german measles.  We have also found that children are not always appropriate due to the private and personal nature of gynecologic exam.

  • Food and beverages

  • Please do not use personal fragrance or perfumes out of respect for our patients who are sensitive to these products.

Cancellation Policy

Our office has implemented a 24 hour cancellation policy. This policy applies to all new patients and existing patients. Our intent is to give each patient the time and attention she deserves. Unanticipated no-shows increase the overall wait time for appointments and disrupt the doctors schedules. Please contact us 24 hours prior to your appointment if you will be unable to attend or our office reserves the right to charge you fee of $25.00 for a missed appointment.

Late Arrivals

Your time is valuable. We strive to stay on schedule throughout the day . If you are late for your appointment, you will affect the other patients in the office. If you arrive more than 15 mins late, you may be asked to wait or reschedule.

Prescription Renewals

Please make sure that we have your current pharmacy information. Prescriptions are renewed during normal office hours. In some instances, we may require that the patient be seen in our office prior to medication renewal. This ensures that your medical information is current and you have not had any complications from the medication. Prescriptions are not renewed by the on-call physician after hours or on the weekends. 

Laboratory Services

These services are provided by Quest Diagnostics. It is the patient’s responsibility to assure that their insurance will cover labs drawn from this lab. Insurance companies often change their preferred laboratory therefore please check your coverage prior to each blood draw. These services are provided for the convenience of our patients.

Financial

All office visits are payable at the time of service. We accept cash, checks, and credit cards (Visa, Mastercard & Discover) and debit cards.  Please contact our billing office should you have any financial questions or concerns.
